Annual Health Update
This form needs to be completed annually by a parent/guardian and submitted to your child's School Nurse at the beginning of the school year for our Health Office records. -
Authorization for Medication in School
Any student requiring medication to be administered at school, both prescription and over-the-counter, must have this form completed and signed by your healthcare provider and parent.

Dental Health Certificate
New York State law (Chapter 281) permits schools to request a dental examination in the following grades: new school entry, K, 2, 4, 7, & 10. Please return the completed form to the School Nurse as soon as possible.
