ATHLETIC PHYSICALS

*Please make sure to bring a completed Sports Physical History Form with you to the appointment (form can be downloaded below)*
Students with prescription glasses or contact lenses should wear or bring them to the physical.
As stated in the eligibility standards, New York State Education Department mandates that all athletes have a physical prior to trying out for a school sport. All Grand Island athletes must have a sports physical by approved medical personnel. An updated physical exam is required every 12 months. Health Exams are valid for 12 months and extended to the last day of that month. For example an exam done on Nov 12, 2021 is valid until Nov 30, 2022. The physical must cover the first day of that sport in order to be valid for that sport. If the physical exam expires during the season, an updated physical is needed for the next season. All physical health exams MUST also be on the NYSED Required Health Exam Form or electronic equivalent printed from your doctor's office.

Sports Physicals may be sent in to the Athletic Department via fax to 716-773-3399, pdf or photos can be emailed directly to the Athletic Trainer at jeffgreen@gicsd.org, or a hard copy may be brought in to the Athletic Office.
When students sign up for a sport, they will be notified if their physical is valid for that season in the Google Classroom set up for each team. A "grade" of 1/1 means it is valid for that season, 0/1 means an updated physical exam needs to be sent in, and "Assigned" means the AT has not yet reviewed it.
Announcements of physical dates are found on this website, emailed to the students, and in the Google classrooms set up for each team.
All students examined more than thirty days prior to the beginning of the sports season must fill out an Interval Health History form. No student will be able to practice without a physical exam and/or health update release.
