you can complete a physical from your own physician and bring in as soon as possible
or call UB Orthopedics at 304-6585 & schedule an appt (approx. cost: $40.00)
All students must bring Sports Physical History Form, signed by parent/guardian. The form can be found by clicking on the "Forms" link in the left column.
Students with prescription glasses or contact lenses should wear or bring them to the physical.
As stated in the eligibility standards, New York State Public High School Athletic Association mandates that all athletes have a physical prior to trying out for a school sport. All Grand Island athletes must have a sport physical by approved medical personnel. An updated physical exam is required every 12 months. Health Exams are valid for 12 months and extended to the last day of that month. For example an exam done on Nov 12, 2020 is valid until Nov 30, 2021. The physical must cover the first day of that sport in order to be valid for that sport. If the physical exam expires during the season, an updated physical is needed for the next season.
Announcement of physical dates are found in this website, a letter mailed home to parent/guardian, posters, coach and announcements in each building prior to the physical date.
Sports physicals can be obtained at:
your primary physician
on-site at school
All students examined more than thirty days prior to the beginning of the sports season must fill out a Interval Health Histiry form. No student will be able to practice without a physical exam and/or health update release.
You can get the necessary forms by clicking on the link to the Grand Island Physical Form link in the left column.
