Athletic Physicals

As stated in the eligibility standards, the New York State Education Department mandates that all athletes have a physical prior to trying out for a school sport. All Grand Island Athletes must have a sports physical by approved medical personnel. An updated physical exam is required every 12 months. Health Exams are valid for 12 months and extended to the last day of that month. For example, an exam done on November 12, 2021, is valid until November 30, 2022. The physical must cover the first day of that sport in order to be valid for that sport. If the physical exam expires during the season, an updated physical is needed for the next season. All physical health exams MUST also be on the NYSED Required Health Exam Form or electronic equivalent printed from your doctor's office.
Sports physicals can be obtained at:

Your primary care physician; and
-
Local urgent care center.
Sports Physicals should be provided to one of the following applicable Health Offices:
High School Health Office, via fax to (716) 773-9049, or a hard copy brought in to the High School Health Office; or
Middle School Health Office, via fax to (716) 773-8841, or a hard copy brought in to the Middle School Health Office.
